Form 5: Application (modified to include heading, signature block and filing party's details as required by Order 41 of the Federal Court Rules)

Rule: Order 4 rule 1
Use: This form is used if you are starting a proceeding in the Court. There are some exceptions to this. Please see the Guide to Form 5 for more details.
Filing fee: If a corporation is liable to pay the fee: $1881.00. In any other case: $785.00. If filing an application for an order for substituted service of a bankruptcy notice: corporation $385.00, in any other case $193.00. If filing an application under HREOC Act: $50. If filing an application and claim of Unlawful Termination of Employment: the fee payable under subsection 644 (2) or (3) of the Workplace Relations Act 1996. Currently this fee is $57.30.

You can apply to have the fee waived if payment would cause you financial hardship.
