Form 4: Conclusion of a document for signature

Rule: Order 41 rule 6
Use: Any document filed, other than an affidavit, annexure or exhibit attached to another document, must be concluded in accordance with Form 4 and signed by the party filing it or by his or her solicitor unless the nature of the document is such that the signature is inappropriate.
