Form 2: Abbreviated entitling of document
Version: 1
Rule: Order 41 rule 1 (4)
Use: This is an abbreviated version of Form 1 and can be used instead of form 1 for any document or form except for an originating process, a document to be served on a person not a party to a proceeding or a final order. In these cases, form 1 should be used. As with form 1, form 2 should be copied/cut or pasted into the top of another document or form if this information is not already there.
